The Clinical Care Manager (RN) leads and coordinates clinical care delivery for home health services, ensuring effective communication and teamwork among care teams, patients, and families. This role supports staff training, promotes a culture of accountability, and optimizes clinical operations to achieve patient-centered outcomes. The position requires an active RN license, home health experience, and strong leadership and communication skills in a compassionate care environment.
