• Medical Lab Tech 5x8hrs Evening Shift 2p-10:30p and rotating weekends.

Job Requirements

• American Heart Association BLS certification is required for all direct patient care personnel.
• License/Certificate Verification must be dated within 30 days of start.
• Trinity Specialty Specific Competency Testing must be completed within a year.
• Trinity Skills Checklist required for clinical contractors.
• Proof of MMR vaccination or IgG titer required.
• Hepatitis B vaccination series required prior to start.
• Proof of Varicella vaccination or IgG titer required.
• Current, non-expired Tdap vaccine required; declinations accepted if no pediatric interaction.
• Physical/Health Statement from exam within last year required.
• Trinity Code of Conduct Form required.
• Trinity I9 form required.
• Trinity Confidentiality of Patient and Proprietary Information and Information System Security Agreement required.
• Trinity Integrity and Audit Services Form required.
• Trinity Conflict Of Interest Disclosure Form required.
• Trinity Criminal Background Check within last 7 years required, dated within 30 days of start.
• Trinity Education Verification required.
• Trinity EPLS report dated within past 30 days required.
• Trinity GSA/OIG report dated within past 30 days required.
• Trinity NSO report dated within 30 days of start required.
• Trinity System Access Form must be completed and uploaded one week before start.
• Trinity Joint Commission CORE Exam within a year required.
• Proof of COVID-19 vaccine booster or declination/exemption required (declination not accepted for CA or IL).
• Proof of Influenza vaccine or declination/exemption required (declination not accepted for CA or IL).
• 10-panel drug screen completed within 30 days of start required.

Schedule Information

• Evening Shift 2p-10:30p, 5 days a week.
• Rotating weekends.
