LanceSoft is seeking a travel nurse RN Care Manager for a travel nursing job in Seattle, Washington.

RN Care Manager - Seattle, WA **This is a HYBRID role (2 days/wk on site, 3 days/wk remote), open to LOCAL and NON-LOCAL candidates,**

- Position / Specialty: RN Care Manager
- Schedule: 5x8hr, M-F, 080-1600 (2 days/week on-site at Beacon Hill clinic, 3 days/week working from home. Clinic will issue all necessary equipment to work remotely.)
- Start Date: ASAP
- Years of experience REQ: 1 year in an Acute Hospital and / or Long Term Acute Care/Rehab/Skilled Nursing setting
- First-timers accepted: Yes
- Weekend REQ: No
- Is on-call REQ? No
- Will this traveler need to float between like units? As Needed Within Skillset
- Locals accepted: Yes. This position is open to LOCAL and NON-LOCAL candidates.

SPECIAL NOTES
- Candidate is expected to have their own reliable transportation (NOT public transportation) for the duration of this contract.
- This Care management role focuses on transitions of care, including care coordination and medication reconciliation, with the goal of reducing ED visits and readmissions.
- All IP discharges, with the exception of labor and delivery and oncology follow-up.
- All ED visits, particular focus on patients with chronic conditions.

FACILITY HIGHLIGHTS
- Type of Facility: Outpatient Clinic (2 days/week on-site at Beacon Hill clinic, 3 days/week working from home. Clinic will issue all necessary equipment to work remotely.)
