Independent Contractor Job Summary The Community Support Worker (CSW) is responsible for conducting patient support activities in accordance with District Health Care Services and Department of Behavioral Health requirements. The CSW provides consumer services that are essential for assisting the consumer in the achievement of rehabilitation and recovery goals as defined in the consumer's Treatment Plan. The CSW is engaged in activities in the community and/or in the consumer's natural setting 90 percent of the time. Independent Contractor Duties and Responsibilities Essential and other important responsibilities and duties may include, but are not limited to the following: Manage a caseload of no more than 40 adults Assist the consumer in the development of interpersonal and community coping skills Assist the consumer in the development of relapse and symptom management strategies and plans Provide and coordinate referrals to make sure that the consumer receives the requested assistance Conducts visits to the consumer's job site, home, hospital room and other natural settings Transport consumers in personal vehicle as needed Transport or coordinate transportation for consumers as needed Adhere to District documentation requirements Turn in billing/progress notes as required Provide crisis intervention to consumers as necessary Meet assigned weekly/monthly productivity requirements (25 billable hours per week) Performs other duties as assigned Independent Contractor Requirements and Qualifications Associates Degree or three years' experience in a relevant human service field One year experience of human service activities Independent Contractor Required Knowledge, Skills and Abilities Strong customer service background Knowledge of medical terminology and insurances Good oral, written, and telephone communication skills Ability to work independently or in a team-oriented environment and interrelate well with individuals with diverse ethnic and cultural backgrounds and needs. Basic working knowledge of computers PHYSICAL EFFORT AND WORK ENVIRONMENT Must be physically able to sit, stand, and walk for long periods of time. Be able to bend, lift, and carry files from one location to another. The position involves the ability to safely operate a motor vehicle to transport oneself, consumers and program supplies as necessary. The physical ability to travel to assigned locations The physical ability to stand, stoop, bend, reach, pull, push, lift, carry objects, to include a human body. The physical ability to perform basic and light home maintenance activities and operate office equipment. The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment Operating office equipment requiring continuous or repetitive hand/arm movements The ability to remain in a sitting position for extended periods. The incumbent shall provide evidence of a recent (within the past twelve (12) months) health assessment that includes a PPD and/or chest x-ray results.
